| A brochure outlining the agency, its programs, its mission and Board of Directors |

Formal applications will be sent or emailed to qualifying organizations by May 2, 2007 and due back to the Foundation by June 1, 2007 (Please let us know if you cannot communicate via email). |

Each organization that receives a formal application also must be visited by Missy MacLeod, between (to be determined) (you must sign up for a visit as soon as you are notified.) |

 |
Grantees will be notified by September 28, 2007. |

The following requests will not be considered: Funding to benefit individual, religious or political activities, fund-raising or travel; legislative or lobbying activities; tuition or scholarships; staff seminars; benefits, competitions, parades or camps.
If you received a Healthy Children's Initiative grant in 2006 you must wait one year before applying for another grant. Also, if you are currently receiving capital funding from the Foundation, we will not consider your request.
